We are currently recruiting for 2 Practice Leaders to join our Supported Living team.

 

About the role

As a Practice Leader you will work as part of a team of managers who are responsible for the day-to-day management of our supported living services. Working alongside people who have a learning and/or physical disability, you will ensure that people receive care and support in line with their wishes and aspirations. You will ensure the delivery of care and support meets the standards as set out by SOL's Values and the National Health and Social Care Standards.  

Main Responsibilities

  • Lead Supported Living teams to ensure they are working with people to develop and maintain relationships outside of paid support and to connect with people in their communities.
  • Work alongside the people we work for and their teams to develop outcome-based support plans and supporting strategies, which are regularly reviewed and actions are conducted
  • Liaise with multi-agency teams to ensure the best possible provision of care and support
  • Work in partnership with other areas of the organisation to ensure the people we support needs are met:
    • SOL Connect supports people to utilise technology-enabled care to enable them to be as independent as they can be.
    • People Team who will ensure the people we support teams are fully staffed and will manage absence within the teams as well as other workforce tasks.
    • Business Support Team around the people we support budgets
  • Ensure rotas are completed in time, in partnership with the person and staff team
  • Set up new services for people wishing to receive support from SOL

Our ideal candidate will be someone who has social care experience and is passionate about empowering the people we work for. If you have not completed your SVQ Level 3 you must be willing to study for this qualification.
